Nursing Diagnoses

Ineffective airway clearance related to bronchospasm
Risk for activity intolerance related to asthma

Goals

Student will maintain clear breath sounds during school hours
Student will self-identify asthma triggers and early warning signs
Student will participate in PE with appropriate accommodations

Interventions

Administer albuterol inhaler PRN per physician order
Monitor respiratory status before and after PE
Ensure quick-relief inhaler is accessible at all times
Notify parent if rescue inhaler used more than 2x/week

Expected Outcomes

Zero emergency room visits for asthma during school year
Student can verbalize 3 asthma triggers by end of semester
Full participation in PE with pre-treatment as needed

Emergency Action Plan

If student shows signs of severe distress (unable to speak in sentences, retractions, O2 sat <92%): 1) Administer albuterol 2 puffs via MDI with spacer, 2) Call 911, 3) Contact parent at 405-555-0234, 4) Stay with student and monitor until EMS arrives.
