Nursing Diagnoses
- Ineffective airway clearance related to bronchospasm
- Risk for activity intolerance related to asthma
Goals
- Student will maintain clear breath sounds during school hours
- Student will self-identify asthma triggers and early warning signs
- Student will participate in PE with appropriate accommodations
Interventions
- Administer albuterol inhaler PRN per physician order
- Monitor respiratory status before and after PE
- Ensure quick-relief inhaler is accessible at all times
- Notify parent if rescue inhaler used more than 2x/week
Expected Outcomes
- Zero emergency room visits for asthma during school year
- Student can verbalize 3 asthma triggers by end of semester
- Full participation in PE with pre-treatment as needed
Emergency Action Plan
If student shows signs of severe distress (unable to speak in sentences, retractions, O2 sat <92%): 1) Administer albuterol 2 puffs via MDI with spacer, 2) Call 911, 3) Contact parent at 405-555-0234, 4) Stay with student and monitor until EMS arrives.
