Nursing Diagnoses

  • Risk for unstable blood glucose level
  • Deficient knowledge (age-appropriate) regarding diabetes self-management

Goals

  • Maintain blood glucose between 80-180 mg/dL during school hours
  • Student will recognize and report symptoms of hypo/hyperglycemia
  • Zero episodes of severe hypoglycemia requiring glucagon at school

Interventions

  • Check blood glucose at 10:00 AM, 12:30 PM, and PRN
  • Administer insulin per sliding scale with meals and high BG corrections
  • Allow unrestricted bathroom and water access
  • Keep fast-acting glucose (juice boxes, glucose tabs) in health office and classroom
  • Communicate with teacher re: signs of hypo/hyperglycemia

Expected Outcomes

  • BG within target range >80% of school checks
  • Student can verbalize hypo symptoms (shaky, dizzy, sweaty)
  • No missed school days due to diabetes management issues

Emergency Action Plan

HYPOGLYCEMIA (<70 mg/dL): Give 15g fast-acting carbs (juice box). Recheck in 15 min. If <54 or unconscious: Administer glucagon 1mg IM, call 911, call parent at 405-555-0301. HYPERGLYCEMIA (>300 mg/dL): Check ketones. If moderate/large ketones: call parent, do not exercise, provide water. Call 911 if vomiting, lethargy, or fruity breath.
