Nursing Diagnoses

  • Risk for injury related to seizure activity
  • Deficient knowledge (family/staff) regarding seizure first aid

Goals

  • Zero seizure-related injuries at school
  • All classroom teachers trained in seizure first aid
  • Medication administered on schedule daily

Interventions

  • Administer Keppra 250mg daily at 10:45 AM
  • Train all teachers and specials staff on seizure first aid annually
  • Ensure no swimming or heights without 1:1 supervision
  • Document any seizure activity: type, duration, recovery time, triggers

Expected Outcomes

  • No seizure-related ER visits during school
  • 100% medication adherence
  • Staff demonstrate competence in seizure response

Emergency Action Plan

SEIZURE: 1) Stay calm, note time seizure started, 2) Clear area of hard objects, ease to floor if standing, 3) Turn on side (recovery position), 4) Do NOT restrain or put anything in mouth, 5) If seizure lasts >5 minutes OR student stops breathing: Call 911 and administer Diastat if ordered, 6) Call parent at 405-555-0378, 7) Document episode.
