Nursing Diagnoses

  • Ineffective airway clearance related to bronchospasm
  • Activity intolerance related to respiratory compromise

Goals

  • Complete daily nebulizer treatment before 10:00 AM
  • Maintain O2 sat >95% throughout school day
  • Reduce rescue inhaler use to <2x/week

Interventions

  • Administer albuterol nebulizer 2.5mg daily at 9:30 AM
  • PRN albuterol inhaler for breakthrough symptoms
  • Monitor O2 sat pre and post treatment
  • Modified PE participation with warm-up period

Expected Outcomes

  • 100% scheduled treatment adherence
  • Zero ER visits for asthma during school
  • Able to participate in modified PE daily

Emergency Action Plan

SEVERE ASTHMA ATTACK: 1) Administer albuterol via nebulizer or 4-6 puffs MDI with spacer, 2) Check O2 sat, 3) If O2 sat <92% or no improvement in 15 min: Call 911, 4) Call parent at 405-555-0578, 5) Keep student upright and calm.
