Nursing Diagnoses
- Ineffective airway clearance related to bronchospasm
- Activity intolerance related to respiratory compromise
Goals
- Complete daily nebulizer treatment before 10:00 AM
- Maintain O2 sat >95% throughout school day
- Reduce rescue inhaler use to <2x/week
Interventions
- Administer albuterol nebulizer 2.5mg daily at 9:30 AM
- PRN albuterol inhaler for breakthrough symptoms
- Monitor O2 sat pre and post treatment
- Modified PE participation with warm-up period
Expected Outcomes
- 100% scheduled treatment adherence
- Zero ER visits for asthma during school
- Able to participate in modified PE daily
Emergency Action Plan
SEVERE ASTHMA ATTACK: 1) Administer albuterol via nebulizer or 4-6 puffs MDI with spacer, 2) Check O2 sat, 3) If O2 sat <92% or no improvement in 15 min: Call 911, 4) Call parent at 405-555-0578, 5) Keep student upright and calm.
